Certificate of Insurance

A current/valid Certificate of Insurance listing Production Zone as an additional insured is needed for every rental.

To ease this process, we suggest the following 3 steps be taken:

1. Fax the above checklist to your Insurance Co. – Insert your company name in space provided on the form.

2. Follow-up with your Insurance Co. to ensure that you have proper coverage (see checklist on print version)

3. Have your Insurance Co. provide a Certificate of Insurance to us that indicates the required coverage.  This can be e-mailed to:

Credit Card Authorization Form
